What Exactly Is Meant by Primary Stability

Primary stability describes how firmly the implant is seated in the bone. This is not a permanent bond, but rather an initial state that forms the basis for further osseointegration.

During this phase, there is not yet a firm biological bond between the bone and the implant. This develops only over time. Initially, therefore, it is only mechanical anchorage that holds the implant in place. The better this initial stability is, the more favorable the conditions for stable osseointegration, because low primary stability can cause the implant to be more sensitive to stress.

Factors Affecting Primary Stability

Whether an implant is stable immediately after placement depends on several factors. These relate to the bone, the type of implant, and the surgical technique.

Key factors include:

  • the density and quality of the jawbone
  • the shape and surface of the implant
  • the precise preparation of the implant site
  • the technique used during placement

In practice, dense bone provides better support than soft bone. For this reason, primary stability is often greater in the lower jaw than in the upper jaw. The shape of the implant also plays an important role, as certain designs have been specifically developed to provide greater stability.

Why Primary Stability Is So Important for Healing

After placement, the body begins to integrate the implant into the bone. This process takes some time and occurs in several stages. Primary stability ensures that the implant remains stable within the bone during this phase.

Regular movement in the implant area can often be problematic, because if the implant is subjected to stress too early or too strongly, it can disrupt the healing process. In unfavorable cases, a stable bond with the bone may not form. Good primary stability reduces this risk. It creates the conditions necessary for the implant to integrate into the bone tissue without disruption.

Measurement and Assessment in Clinical Practice

In clinical practice, the stability of an implant is not assessed solely on a subjective basis. There are various methods available to evaluate it more precisely.

Typical methods include:

  • Measuring the insertion torque during placement
  • specialized devices for determining implant stability
  • Clinical assessment by the treating dentist

These values are important for planning further treatment. For example, they provide information on whether an implant can be loaded early or whether a longer healing phase is advisable.

Relationship Between Primary Stability and Loading

An important consideration in implantology is determining when an implant can be loaded. Primary stability plays a decisive role in this regard. If stability is good, a prosthesis can sometimes be placed earlier. However, this does not mean that this is always advisable, as the decision depends on several factors and is made on a case-by-case basis.

If primary stability is lower, more time is usually allowed for healing. This ensures that a stable bond with the bone can develop.

Limitations of Primary Stability

Although primary stability is an important factor, it is merely the starting point. Later on, biological integration becomes crucial. Only once the bone has firmly bonded with the implant is it permanently stable.
