Causes of Bone Loss

Studies have shown that bone loss most commonly occurs as a result of tooth loss. Without daily chewing, the bone is no longer sufficiently stimulated and is therefore gradually broken down by the body. This often begins shortly after a tooth is lost or extracted, and the rate of bone loss varies from person to person. Inflammatory diseases such as periodontitis accelerate this bone loss. However, tooth root infections and surgical procedures can also significantly impair bone quality. Sometimes, ill-fitting dentures also contribute to further bone loss due to excessive pressure.

The Goal of Bone Augmentation

The goal of bone augmentation is to create sufficient bone volume to establish a stable structure for implant placement. It is not only the amount of bone mass that is built up that matters, but also its quality. The regenerated bone must be stable enough to withstand the mechanical stresses of chewing over the long term. At the same time, it must allow for stable integration of the implant. Careful planning is therefore necessary to select the appropriate technique and material.

Horizontal and Vertical Augmentation

There are two types of bone augmentation:

Horizontal procedure— A horizontal augmentation is performed when the alveolar ridge is too narrow for an implant. In this case, it must be widened.

Vertical procedure – A vertical augmentation is performed when the bone height is insufficient. This technique is significantly more challenging, as the bone is built up against the natural direction of stress. Often, both procedures must be combined to achieve sufficient bone volume.

Materials for Bone Augmentation

Various materials are available for bone augmentation. Autogenous bone is frequently used because it integrates well with existing structures. Furthermore, this procedure exhibits high biological compatibility.

In addition, bone substitute materials are used. They help the body form new bone. After a while, they are replaced by tissue. However, a combination of both materials is usually used.

Bone Augmentation in the Upper Jaw

In the upper jaw, the available bone in the posterior region is often limited by the maxillary sinus. When a tooth is lost, the sinus can expand further. This further reduces bone height, making it necessary to perform bone augmentation beneath the maxillary sinus. This creates space for new bone to form.

Timing of Implant Placement

The timing of implant placement depends on the extent of the bone grafting. For smaller defects, implants can often be placed at the same time as the bone grafting. For more extensive procedures, however, it is best to wait until the body has healed from the procedure. During this time, the graft material is integrated into the bone.

Healing and Biological Processes

Bone regeneration follows bone grafting. During this process, the graft material is gradually converted into the body’s own bone. However, this process takes time and depends on various factors. These include blood flow and the patient’s individual healing capacity. Sufficient stability of the newly formed bone is crucial for the implant to remain securely in place.

Risks and Possible Complications

As with any surgical procedure, bone grafting also carries certain risks. These include infections, impaired wound healing, or insufficient integration of the graft material. However, these risks can be significantly reduced through careful planning and the use of an appropriate technique. Good postoperative care helps prevent complications.

Bone augmentation significantly expands the possibilities of implantology. Even in unfavorable initial conditions, a stable restoration can be achieved. A strong bone base is essential for implants to remain secure and function properly. Bone augmentation therefore provides a crucial foundation for achieving good long-term results.
